Provider Credentials

Families are searching for providers who know what they’re doing. A provider credential from The Synergy Academy tells them you trained with us, showed what you learned, and committed to delivering these therapies the way they are taught.

We offer two: RestOralase Provider for the laser work, and NeuralFORM Device Provider for appliance therapy. Earn one or both. Each takes three steps.

1. Learn

Complete the NeuralFORM Aware Series, then the hands-on course for your discipline. Aware gives you the neurology and the reasoning. The hands-on course puts it in your hands.

2. Show

Pass two online exams: the proctored Aware exam, right after your Aware weekend, and the exam for your discipline, where 80% passes. Both stay open to you whenever your TSA membership is active, so take them while the material is fresh.

3. Commit

Sign the Provider License Agreement. It gives you permission to use our registered names to describe your services, and it asks you to deliver the protocols as taught, within the scope of your license. It’s how we keep the names meaning something, for you and for the families who trust them.

Choose your credential

• RestOralase Provider (RestOralase®, including the OraLase® foundation). Course: RestOralase hands-on course, 22 CE hours. Who can earn it: dental, medical, physical therapy, chiropractic, and speech-language pathology providers, where your state permits Class IV non-ablative laser use. Laser safety training and access to a qualifying laser required.
• BabyLase® (infants, ages 0–2). Course: BabySteps (virtual) and BabyBuilding, the hands-on lab, added to the RestOralase course, 6 more CE hours (28 total). Who can earn it: RestOralase Providers. Separate BabyLase exam plus 10 documented infant cases.
• NeuralFORM Device Provider (NeuralFORM GROW™ and NeuralFORM Enhance™). Course: NeuralFORM Dental Device Therapies hands-on course, 12 CE hours. Who can earn it: providers with an active dental license.

Earn both and you list both: NeuralFORM Device Provider and RestOralase Provider. No extra course or exam.

Inside the NeuralFORM Device Provider credential

One course, one credential, two directions of care.

• NeuralFORM GROW™ (Guided Reflexive Orofacial Wellness) supports healthy growth and development.
• NeuralFORM Enhance™ supports the maintenance of neural circuits and structural retention, including the Synergy AIR, NINJA and ALF for Trauma Release designs.

Synergy ALF can serve either one, depending on what the patient needs.

Keeping it current

Your credential is good for two years. To renew, document 10 hours of relevant continuing education and confirm your license is active. Your license agreement renews with it.

Already trained with us?

• If you hold a Fellowship or Mastership from The Synergy Academy, it carries over to the matching credential. Pass the one-time Update exam and sign the license agreement, and you’re set.
• If you trained before 2027, the Update exam brings you current with what the literature review changed. It’s one time, not a recurring requirement.
• If you trained with us before 2027, sign the Provider License Agreement by June 30, 2027 to keep using our registered names to describe your services.
• Trained in OraLase somewhere else? Document your training, complete the course content we require, and pass the exam. Contact us and we’ll walk you through it.

Our public provider directory opens in 2027. Every credential holder is listed by credential and the date earned, with no extra fee. We maintain it ourselves, so families can trust what they find.

What this credential is, and isn’t

Both are training credentials from The Synergy Academy. They are not specialty recognition, board certification or a substitute for licensure. Every provider practices within the scope of their own license.

Where it started

The ALF appliance and the OraLase sequence were created by Dr. Darick Nordstrom. NeuralFORM, RestOralase, BabyLase, NINJA and the SWITCH™ test were developed by Dr. Angie Tenholder and are taught through The Synergy Academy.
